I have shares in CUB but not received application form for applying?

Whom should I get in touch with for either getting application or can I pay the money to appointed bank by giving the DP-ID & my ID.

Yes Rights Issue application is send to shareholders, and I have received the same.

Kindly advice on right issue by City Union Bank. Should I go for it or not. Offer opens on 2nd of December and closes on 9th.

If the same is good is it possible for me to get more share than offered.
